Handle the case where microcode gets loaded on the BSP's hyperthread
sibling first and the boot_cpu_data's microcode revision doesn't get
updated because of early exit due to the siblings sharing a microcode
engine.
For that, simply write the updated revision on all CPUs unconditionally.

When preparing an MCE record for logging, boot_cpu_data.microcode is used
to read out the microcode revision on the box.
However, on systems where late microcode update has happened, the microcode
revision output in a MCE log record is wrong because
boot_cpu_data.microcode is not updated when the microcode gets updated.
But, the microcode revision saved in boot_cpu_data's microcode member
should be kept up-to-date, regardless, for consistency.
Make it so.

After updating microcode on one of the threads of a core, the other
thread sibling automatically gets the update since the microcode
resources on a hyperthreaded core are shared between the two threads.
Check the microcode revision on the CPU before performing a microcode
update and thus save us the WRMSR 0x79 because it is a particularly
expensive operation.

Since on Intel we're required to do CPUID(1) first, before reading
the microcode revision MSR, let's add a special helper which does the
required steps so that we don't forget to do them next time, when we
want to read the microcode revision.

Currently, x86_cache_size is of type int, which makes no sense as we
will never have a valid cache size equal or less than 0. So instead of
initializing this variable to -1, it can perfectly be initialized to 0
and use it as an unsigned variable instead.

Commit b94b73733171 ("x86/microcode/intel: Extend BDW late-loading with a
revision check") reduced the impact of erratum BDF90 for Broadwell model
79.
The impact can be reduced further by checking the size of the last level
cache portion per core.
Tony: "The erratum says the problem only occurs on the large-cache SKUs.
So we only need to avoid the update if we are on a big cache SKU that is
also running old microcode."

Instead of blacklisting all model 79 CPUs when attempting a late
microcode loading, limit that only to CPUs with microcode revisions <
0x0b000021 because only on those late loading may cause a system hang.
For such processors either:
a) a BIOS update which might contain a newer microcode revision
or
b) the early microcode loading method
should be considered.
Processors with revisions 0x0b000021 or higher will not experience such
hangs.

Thomas Voegtle reported that doing oldconfig with a .config which has
CONFIG_MICROCODE enabled but BLK_DEV_INITRD disabled prevents the
microcode loading mechanism from being built.
So untangle it from the BLK_DEV_INITRD dependency so that oldconfig
doesn't turn it off and add an explanatory text to its Kconfig help what
the supported methods for supplying microcode are.

Set the initrd @start depending on the presence of an initrd. Otherwise,
builtin microcode loading doesn't work as the start is wrong and we're
using it to compute offset to the microcode blobs.

Make CONFIG_MICROCODE a bool. It was practically a bool already anyway,
since early loader was forcing it to =y.
Regardless, there's no real reason to have something be a module which
gets built-in on the majority of installations out there. And its not
like there's noticeable change in functionality - we still can load late
microcode - just the module glue disappears.

We do not check the input data bounds containing the microcode before
copying a struct microcode_intel_header from it. A specially crafted
microcode could cause the kernel to read invalid memory and lead to a
denial-of-service.

"Here's the big driver core and sysfs patch set for 3.14-rc1.
There's a lot of work here moving sysfs logic out into a "kernfs" to
allow other subsystems to also have a virtual filesystem with the same
attributes of sysfs (handle device disconnect, dynamic creation /
removal as needed / unneeded, etc)
This is primarily being done for the cgroups filesystem, but the goal
is to also move debugfs to it when it is ready, solving all of the
known issues in that filesystem as well. The code isn't completed
yet, but all should be stable now (there is a big section that was
reverted due to problems found when testing)
There's also some other smaller fixes, and a driver core addition that
allows for a "collection" of objects, that the DRM people will be
using soon (it's in this tree to make merges after -rc1 easier)
All of this has been in linux-next with no reported issues"

We've grown a bunch of microcode loader files all prefixed with
"microcode_". They should be under cpu/ because this is strictly
CPU-related functionality so do that and drop the prefix since they're
in their own directory now which gives that prefix. :)
While at it, drop MICROCODE_INTEL_LIB config item and stash the
functionality under CONFIG_MICROCODE_INTEL as it was its only user.
